Add Health Measurement Details for Patient

1. Go to 'Queue'
   

1. there are multiple ways to enter medical record -> here, The simplest way to go to medical record is to have the patient already in queue
   

1. On the far right click on 'health measurement'
   

1. Enter in patient 'height', 'weight', 'waist' and smoking details, 'bmi' is automatically calculated
   

1. click on 'save' once done to save changes
